Our Leadership Team:

Oversees the activities within their site that are being led by the IES to support and facilitate progress

Shares ongoing site initiatives and discusses solutions to challenges that were encountered administratively (e.g. staff, finances, etc.) in collaboration with all Consortium members

Contributes to the ongoing evolution of the Consortium through decision-making and consensus seeking
Our Implementation & Evaluation Specialist Team:
-
Leads and participates in the development and implementation of activities and best practices through the application of implementation science
-
Forms and engages SIT teams
-
Supports SIT team throughout all stages of implementation to optimize success and sustainability
-
Communicates implementation concerns and challenges to site leads to facilitate decision-making
-
Liaises with IESs to address site specific implementation issues
-
Shares learnings encountered during the implementation process with the Consortium IES team
-
Completes cross-site implementation profiling

Our Patient Advisory Panel:
-
Is composed of patient advisors from across Canada that bring their lived expertise and perspectives to this role, while participating in collaborative conversations related to a national quality improvement initiative
-
Shares their views and knowledge on patient engagement, provides feedback on current processes, identifies potential problems and barriers, and contributes to the design of effective strategies for community follow-ups
-
Provides feedback and gives context to the interim results using their own backgrounds and communities’ perspective
-
Contributes to plans for communicating the results with target audiences
